Case Management
Case Management is a Senior Connect service where we work with low-income individuals 55+ in our service area to help aging in place and promote independence. We will help identify and meet personal goals. SIL supports seniors in making informed decisions, so they can transition from unnecessary dependency to their fullest inclusion in our community.
