Consumer Directed Services (CDS) in Missouri

What Are Consumer Directed Services?

Consumer Directed Services (CDS) is a Missouri Medicaid program that allows people with disabilities to hire and manage their own personal care attendants. The goal is to help people live independently in their own homes and communities, rather than in institutions.
In the CDS program, YOU direct your own care. You are the employer, and the attendant of your choosing is your employee. SIL has offered personal care services since 1984. Our number one goal is helping you maintain your independence. Your choice matters!

Application process: Individuals interested in receiving CDS must have an assessment completed by the Division of Senior and Disability Services (DSDS) to determine if you are eligible and, if so, what type of service you need. The assessment is free and can be scheduled by calling 866-835-3505, a toll-free number, and request to be assessed for CDS by DSDS staff. Have your Medicaid number ready. 

Who Is Eligible for CDS in Missouri?

To qualify for Consumer Directed Services in Missouri, you must:

  • Be 18 years or older

  • Be a Missouri resident

  • Be Medicaid-eligible

  • Have a physical disability

  • Be able to direct your own care

  • Require assistance with daily living activities like bathing, dressing, or meals

  • You meet the level of care through a needs assessment with DSDS
SIL will serve as the Fiscal Agent. Our staff will assist you with the completion of all the paperwork and handle all tax withholdings and payments to the IRS, Missouri DOR, Employment Security, and Social Security. The CDS program is regulated by the Missouri Department of Health and Senior Services (DHSS).

  • Can I hire a family member through CDS?
    Yes, as long as they are not your spouse or legal guardian.

  • Is there a cost to participate in CDS?
    No, if you are Medicaid-eligible.

  • How long does it take to get started?
    Most applicants can begin services within 30–45 days.

How to Apply for Consumer Directed Services

  1. Contact Services for Independent Living to start the intake process

  2. Complete an in-home assessment through the Department of Health and Senior Services

  3. Hire your attendant — this can be a friend or family member (excluding a spouse)

  4. Start receiving services

In Home Services

Similar to CDS, In Home Service attendants are trained and employed by SIL. You can sign up for In Home Services with Private Pay, Medicaid, or Veterans Administration, based on eligibility.
